Laser treatments such as CO2 and Erbium skin resurfacing lasers revolutionized the way cosmetic surgeons treated wrinkles from the very dangerous and unpredictable deep chemical peels such as the phenol peel to a more precise, predictable and safer method. However these lasers were associated with a painful and lengthy recovery period that was fraught with risks and complications. Fractional laser technology has dramatically improved such ablative laser treatments and other non-ablative laser treatments and eliminated the prolonged and uncomfortable recuperation associated with the non-fractionated lasers.

The Fractionated Laser Advantage
Lasers that are fractionated induce their effect as a microscopic grid skipping and sparing the untreated skin in within the treatment grid. The net effect of this method significantly reduces downtime to a few days by allowing faster skin healing from the untreated skin areas and minimizes adverse effects while yielding dramatic skin rejuvenation results. Fractionated laser treatments do not require general anesthesia as they are well tolerated with or without topical anesthetic creams.

Ablative Fractionated Lasers
CO2 and Erbium skin resurfacing lasers ablate and remove the top layers of skin to treat wrinkles (rhytides) and sun damaged skin (photo aging). The skin will look pink and scabbed and will peel and heal on the skin surface with in 4 to 14 days. The internal skin healing consisting of collagen regeneration occurs over the next 3 to 6 months.

Non-Ablative Fractionated Lasers
Nonablative technologies including infrared lasers (Titan), intense pulsed light (IPL, photofacial), and radiofrequency devices (Thermage), have introduced nonsurgical options to rejuvenating and tightening skin without down time and with minimal risk.

Many diet fads have come and gone without much success of a long-term reduction in weight.  For many who try diets and are unsuccessful to lose weight, medical causes of such weight gain must be considered.  In some individuals liposuction can be used to de-bulk their fat in order to help them lose weight and to motivate them to maintain their new body shape.  In others liposculpture can be utilized to remove genetically predisposed pockets of fat that seem not to melt despite rigorous diet and exercise. 

 

Medical Weight Management

Medical causes of weight gain mainly occur because of an imbalance in the hormone system.  Most commonly associated medical conditions are Hypothyroidism, Polycystic Ovarian Syndrome (PCOS), and Insulin Resistance Syndrome (IRS), all of which can be tested for with the appropriate blood test and treated with medications.

     As the bikini season approaches my patients seek to improve the shape of their bodies as to not be self-conscious about their figures.  Women tend to have genetically predisposed of fat collection that is usually not resolved with diet and exercise alone.  The most common of these areas are the outer thighs where fat accumulates and gives a saddlebag appearance that even in cloths is not flattering. Other areas include the fat collection under the buttocks, on the back of the thighs, called the "Banana Fold," the inner thigh as well as the knee area.   When fat accumulates in the areas, the buttocks appear undesirably wide and the skin on the thighs and buttocks show dimples, indentations and cellulite.  In order to achieve a well-proportioned contour to the body liposculpting, cellulite correction, mesotherapy (lipodissolve), and liposhifting methods can be used to sculpt the thighs, buttocks and knees. 

     Liposuction alone will not necessarily yield a desired outcome.  Liposculpting techniques must be utilized to contour the thighs and buttocks, while the cellulite correction methods may need to be used to smooth the skin.  The secret to achieving a beautifully contoured silhouette is to sculpt the body in a proportionate manner to create, a smooth natural curves that define and accentuate these features.

HIPS AND SADDLE BAGS

Hips and saddlebags on the outer thighs need to be sculpted to yield a smooth slender curve from the hips and to the knees.  Some patient's figures will need liposuction to remove fat from the bulging areas along with liposhifting of fat into depressed areas to attain the desired figure.

INNER THIGHS

The inner thighs should not be touching each other when the feet are at their normal distance for standing at ease.  The inner thighs must be contoured carefully to achieve this, while maintaining the natural curvature or the inner thigh.          

BANANA FOLDS

The banana folds are the fat pockets on the back of the thighs just below the buttocks.  To have the best results, this fat must be removed and the area sculpted in a very specific manner.  This area requires technical finesse and skills of an expert liposculpture to shape the buttock and thigh in an optimal manner.  Sculpting this area will improve the shape of the thighs and buttocks.   

THE BUTTOCKS

There are different shapes to buttocks depending on one's genetic makeup and personal habits.  The buttocks can be sculpted to be rounder, smaller, or plumper.  If there is excess lateral thigh fat, the buttocks will appear wide and    ill defined. Liposculpture of the lateral thighs, banana folds area under the buttocks and sometimes the lower back area as well, will defined and bring out a beautifully shaped buttocks. 

KNEES

The knees tend to accumulate fat and knob with time.  Liposuction was developed to remove such fat pockets and to shape the area.

FRONT AND THIGHS

The front of the thighs must be kept in proportion to the rest of the thigh.  If the inner and outer thighs are aggressively liposuctioned in a fuller thigh, the front thighs will appear to protrude unnaturally.  If the anterior thighs need to be liposculpted, it must be done by an experienced liposuction doctor since it is a very technically challenging area. 

CELLUITE CORRECTION

Liposculpture will not treat cellulite.  There are different kinds of cellulite and there are different surgical and nonsurgical methods that are utilized to correct cellulite.  These methods should be combined with liposculting techniques for the best results.

ANESTHESIA

Most liposculpting and cellulite correction procedures are done safely with the tumescent technique local anesthesia.  General anesthesia can be given for larger cases or for patients who desire it.

RECOVERY

After the procedure, patients experience soreness that is similar to that of muscle soreness after a hard workout that does not require pain medication. Most patients are able to resume daily activities the next day after surgery and exercise within 2 weeks.

MILLIONS OF PROCEDURES PERFORMED

The annual report released last month by The American Society for Aesthetic Plastic Surgery indicates that 11.7 million cosmetic procedures were performed in the US in 2007 at a cost of roughly $13.2 billion.

The popularity of cosmetic procedures exploded in the last decade. Since 1997, surgical procedures have increased by 114 percent, while nonsurgical procedures rose by an impressive 754 percent, according to the report.

Eighteen percent of the procedures performed in 2007 were surgical and 82 percent were nonsurgical.

The most popular cosmetic surgical procedure last year was liposuction, with nearly half a million operations performed. Rounding out the top five were breast augmentation, eyelid surgery, abdominoplasty (better known as a tummy tuck), and breast reduction.

Botox injections were far and away the most popular nonsurgical procedure with nearly 2.8 million treatments logged. Hyaluronic acid injections were the next most popular with roughly half the number of Botox procedures. Other procedures making the top five were laser hair removal, microdermabrasion, and laser skin resurfacing.

The vast majority of patients undergoing cosmetic procedures were women, the report indicates. Ninety-one percent of the procedures (10.6 million) performed in 2007 were on female patients.

Compared with 2006, however, there was a much greater increase in cosmetic procedures among men than women: 17 percent vs. 1 percent.

SIGN SPOTS MOST ALLERGAN GETS SUBPOENA ON BOTOX PROMOTION
Allergan Inc said last month that it received a subpoena from the Department of Justice investigating the promotion of Botox, a drug best known for its cosmetic use. The drug maker said authorities were seeking documents regarding promotional, educational and other activities relating to Botox. Allergan said it believes the subpoena is related to alleged off-label promotion of Botox to treat headaches. Botox, which is best known for smoothing facial wrinkles, is not approved as a headache treatment. The subpoena comes a few weeks after regulators said they were reviewing the safety of Botox.


EARPLUGS NEW FASHION ITEM
Earplugs may not be the most glamorous fashion item around, but a charity for the deaf has asked designers to pitch suggestions to make them hip enough for young clubbers at risk of long-term hearing damage. The Royal National Institute for the Deaf (RNID) says 90 percent of young clubbers have experienced early signs of hearing damage.
